• Registration forms
• Up to date CV
• Valid passport (and RTW if overseas passport)
• Driving licence (provisional or full licence)
• Birth certificate
• 2 x Recent proof of address
• Proof of National Insurance (NI) number
• Current enhanced DBS
• 2 x Clinical referees from current and most recent posts
• Serology and immunisations report
• Indemnity
• Most recent appraisal documents
• Proof of Profession:
• Statement of entry to the register
• Qualification certificates
• HCPC/NMC/GMC registration and pin number
• Performers list registration letter and CCT certificate
